COVID-19 SAFETY INFORMATION:

By attending this Seacrest Foundation (“Foundation”) Women’s Auxiliary Gala (“Gala”), I/we certify that:

I/we am/are fully vaccinated against COVID-19

OR

that I/we will provide negative PCR test results obtained within 72 hours prior to the event. I/we will present these results at entry to Gala.

In light of the ongoing spread of COVID-19, individuals have a duty to self-monitor, and those who believe that they may have been exposed to a confirmed or suspected case of COVID-19, who have come into contact with anyone who has COVID-19 or COVID-19 symptoms in the past ten (10) days (prior to the event date), or who have been diagnosed with COVID-19 and are not yet cleared as noncontagious by state or local public health authorities or the health care team responsible for their treatment, are not permitted to attend the Gala. 

ASSUMPTION OF THE RISK:

I/we acknowledge and understand the following: 

1. While participating in the Gala, the Foundation cannot guarantee that its participants, volunteers, partners, or others in attendance at the Gala will not become infected with COVID-19. 
2. I/we knowingly and freely assume all such risks related to illness and infectious diseases, such as COVID-19, even if arising from the negligence or fault of the Foundation and its affiliated partners and sponsors, including in each case, without limitation, their directors, officers, employees, volunteers, and agents (the “Released Parties”); and
3. I/we hereby knowingly assume the risk of injury, harm and loss associated with the Gala, including any injury, harm and loss caused by the negligence, fault, or conduct of any kind on the part of the Released Parties.

LIABILITY WAIVER AND RELEASE OF CLAIMS:

I/we hereby release, waive and forever discharge any and all liability, claims, and demands of whatever kind or nature against Released Parties, either in law or in equity, to the fullest extent permissible by law, including but not limited to damages or losses caused by the negligence, fault or conduct of any kind on the part of the Released Parties, including but not limited to death, bodily injury, illness, economic loss or out of pocket expenses, or loss or damage to property, which i/we, my/our heirs, assignees, next of kin and/or legally appointed or designated representatives, may have or which may hereinafter accrue on my behalf, which arise or may hereafter arise from my/our participation with the gala.

ACKNOWLEDGEMENT:

I/we acknowledge that I/we have read and understand the above and agree to abide by its content. I/we recognize that the Foundation and its affiliates are responsible for the wellbeing of many frail and high risk individuals and understand that my/our actions impact many people.
