COVID-19 SAFETY INFORMATION:

By attending this Seacrest Foundation (“Foundation”) event, I/we acknowledge that:

It was requested that I take a COVID-19 Rapid Antigen test on the day of the event, October 24, 2022, prior to arrival.

In light of the ongoing spread of COVID-19, individuals have a duty to self-monitor, and those who believe that they may have been exposed to a confirmed or suspected case of COVID-19, who have come into contact with anyone who has COVID-19 or COVID-19 symptoms in the past ten (10) days prior to the event date, or who have been diagnosed with COVID-19 and are not yet cleared as noncontagious by state or local public health authorities or the health care team responsible for their treatment, are asked to be responsible in caring for the safety of others when making a decision to attend.

ASSUMPTION OF THE RISK:

I/we acknowledge and understand the following: 

1. While participating in the event, the Foundation cannot guarantee that its guests, participants, volunteers, partners, or others in attendance at the event will not become infected with COVID-19 or sustain an injury of any kind.
2. I/we knowingly and freely assume all such risks related to illness and infectious diseases, such as COVID-19, even if arising from the negligence or fault of the Foundation and its affiliated partners and sponsors, including in each case, without limitation, their directors, officers, employees, volunteers, and agents (the “Released Parties”); and
3. I/we hereby knowingly assume the risk of injury, harm and loss associated with the event, including any injury, harm and loss caused by the negligence, fault, or conduct of any kind on the part of the Released Parties.

LIABILITY WAIVER AND RELEASE OF CLAIMS:

I/we hereby release, waive and forever discharge any and all liability, claims, and demands of whatever kind or nature against Released Parties, either in law or in equity, to the fullest extent permissible by law, including but not limited to damages or losses caused by the negligence, fault or conduct of any kind on the part of the Released Parties, including but not limited to death, bodily injury, illness, economic loss or out of pocket expenses, or loss or damage to property, which I/we, my/our heirs, assignees, next of kin and/or legally appointed or designated representatives, may have or which may hereinafter accrue on my behalf, which arise or may hereafter arise from my/our participation with the event.

ACKNOWLEDGEMENT:

I/we acknowledge that I/we have read and understand the above and agree to abide by its content.

I/we further recognize that the Foundation and its affiliates are responsible for the wellbeing of many frail and high-risk individuals and understand that my/our actions, especially with regard to COVID-19 precautions, impact many people.
